CO-MAT Continuity of Care Meetings Impact Report (2024-2025)

The 2024-2025 CoC meetings were a collaboration between the Colorado Consortium for Prescription Drug Abuse Prevention, North Colorado Health Alliance, and Denver Health's Center for Addiction Medicine (CAM) Academy. This report provides an overview of findings from the regional meetings aimed at improving coordination between jails, healthcare providers, and community organizations to support continuity of care after release.
